Human Resources: Lake County Government Notices and Forms 2025-26

Lake County Government is pleased to announce the availability of the 2025-26 Annual, New Hire, and Other Notices and Forms document. This important resource provides employees and their families with critical information about health insurance benefits, enrollment rights, and compliance notices required by federal and state law.

Key Information Included

The document contains several essential notices that employees should review carefully:

Premium Assistance Programs (CHIPRA/CHIP Notice): Employees and dependents who are eligible for Medicaid or CHIP may qualify for premium assistance programs that can help pay for employer-sponsored health coverage. Indiana residents can contact the Health Insurance Premium Payment Program at 1-800-403-0864 or visit in.gov/medicaid for more information.

Women's Health and Cancer Rights Act: The Lake County health plan provides benefits for mastectomy-related services, including all stages of reconstruction, prostheses, and treatment for complications. Contact the plan administrator at 219-755-3211 for details.

Wellness Program: The Lake County Government Blood Draw is a voluntary wellness program available to all employees, administered in accordance with federal regulations including the ADA and HIPAA. All personal health information is kept confidential and maintained separately from personnel records.

Health Insurance Marketplace Options: Employees may have coverage options through the Health Insurance Marketplace. The document explains eligibility for premium tax credits and special enrollment periods.

Special Enrollment Rights: Employees who decline enrollment due to other coverage may be able to enroll later if that coverage ends. New dependents from marriage, birth, or adoption also trigger special enrollment opportunities within 30 days of the qualifying event.

COBRA Continuation Coverage: Detailed information about continuation coverage rights is provided for employees and dependents who may lose coverage due to qualifying events such as termination, reduction of hours, divorce, or loss of dependent status.

The U.S. Treasury Department (the “Treasury Department”) awarded Lake County, Indiana (the “County”) $94,301,324 in Recovery Funds. On June 15, 2021, Treasury Department advanced $47,150,662 and on June 21, 2022, Treasury Department advanced $47,150,662 of Recovery Funds to the County. The Recovery Funds allocated to the County represent an unprecedented opportunity for the County to make transformative investments in equity to emerge from the COVID-19 pandemic in an even better place to live, work, and raise a family.
